	Community Nursing Team

	The Community Nursing Team, who are Mandy Gundry, Phillippa Varnham and Maggie Bryant are based at the Farnham Hospital and Centre for Health. They work very closely with the Practice Team, visiting the Surgery on a daily basis. Their contact number is 01483 782069

	The Health Visiting Team Based at Farnham Hospital and Centre for Health. The Health Visiting Service is being provided by a team approach. Contact number : 01483 782088

	Midwives and Maternity Care In this area we are able to use both the Frimley Park Hospital and Royal Surrey County Hospital Maternity Facilities. Midwives from these two hospitals attend our surgery to deliver ante natal care. Contact numbers: Frimley Park Hospital Midwife, contact the Community Midwives on0 01276 604241 Royal Surrey County Hospital Midwife on 01483 571122 Jane Monk Community Midwife for the Royal Surrey County Hospital may be contacted direct via e mail for access to maternity services Jane.monk@nhs.net

	Community Mental Health Team Services are provided according to age: For under 18 years of age and children in full time education, care is provided by the Child and Adolescent Mental Health Service. Contact: 01483 782000 Aged 18 - 65 years by the Community Mental Health Team (Adult) 01483 7820095 Over 65 by the Community Mental Health Team ( Elderly) 01483 782070 Referral to the above teams can be discussed with your GP. Counselling and Psychology Services are provided by Guildford and Waverley Primary Care Trust and can be accessed via your GP.
