The aim of the Downing Street Practice Patient Forum is to encourage patients of the practice to join discussions and debates with the partners and staff in order to improve the delivery of health services in their area.
To do this the Downing Street Practice Patient Forum will:
- Facilitate discussions between the patients and the practice by creating an email address for patients aged over 16 years to use to share their ideas and receive information about developments at the practice and local health services This is referred to as the Virtual Group and the email address is firstname.lastname@example.org
- Hold open forums for all patients and the partners and practice staff to share their views and ideas to improve the delivery of services and work together to promote health education for all patients at the practice
- Act as a channel of communication between the patients of the Practice and the North East Hants & Farnham Clinical Commissioning Group who hold the budget for purchasing health services locally

Click here to complete the "Friends and Family" survey and let us know how you felt about your recent visit to the surgery.
We aim to gather patients from as broad a spectrum as possible to get a truly representative sample of their views. We need young people, workers, retirees, people with long term conditions and people from ethnic minorities.
We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.
If you would like to consider joining either of these groups, please e-mail email@example.com or look at the notice board in the surgery (please note this e-mail is manned by one of our patient group members and not by the surgery administration).
