You can also choose to take 3 further steps, designed to make your interactions with the Practice easier.

Once we have registered you, we will send you a text telling you the GP who is responsible for your care.  That text will also invite you to text back your preferences for the items below.  If you prefer, just email the information when you submit your forms, to

  1. Send us your photo ID so that we can link your new record to the NHS App.  You can use it to make appointments, order prescriptions, see blood results and view your medical record.
  2. Tell us your preferred pharmacy where we will send your prescriptions.
  3. Tell us if you want to opt out of sharing your information with other Healthcare services (if you do not tell us, we will share your basic information).  See Summary Care Record
